Care Management

Our Care Managers conduct home visits with seniors to evaluate their needs and determine appropriate services. The goal is to help older adults live as independently as possible. Typically, Care Management is provided for seniors that qualify for our in-home services (Personal Care, Homemaker, Respite) or Meals on Wheels. To qualify for in-home services, the adult must be 60 years of age or older, reside in North Fulton and have a chronic need that requires assistance.

Personal Care Assistance

Personal Care Assistance is provided by a Certified Nursing Assistant (CNA) to help with bathing. The exact number of hours that are provided is based on our Care Management assessment. Typically, clients are provided Personal Care Assistance for one hour, two or three times a week.

Homemaker Services

Homemaker Services are light housekeeping services provided to assist with tasks that have become physically difficult or risky. Homemaker Services are typically provided by a Certified Nursing Assistant (CNA), and may include the following light housekeeping duties: vacuuming, sweeping, mopping floors, cleaning the bathroom, laundry, bagging and taking out trash, making the bed and/or changing the linens and assisting with organizing household routines. The exact number of hours provided is based on our Care Management assessment. Typically, clients are assisted with Homemaker Services once a week.

Respite Care

Caring for a family member, spouse, partner or friend can often be overwhelming for the caregiver. This leads to stress for both the caregiver and the person receiving care. Although Homemaker or Personal Care services provide assistance, there may be times when Respite Care is offered for families when their loved one cannot be left alone. Respite Care is a set schedule of time for the regular caregiver to tend to their needs, and a professional will come to stay with their loved one. The professional, typically a Certified Nursing Assistant (CNA), can provide companionship, help with leisure activities, simple meal preparation and assistance with getting around the home. The exact number of hours that are provided is based on the Care Management assessment. Typically, caregivers are provided with Respite for up to eight hours a month.
